Every official form ask for your forename(s) and surname (or family name).
As I've never been an xian, I've never had a xian name.

Archaic Anglo-Saxon convention. Quaint but dated term for that
individual label qualifier, the "First Name". How else would you tell
all the John Smiths apart? (other than the bar code tattooed on the
forehead) :)
That reminds me of the old Welsh flavored joke which ends with the
punchline; "Oh! You mean Jones the spy."
If you check US Government forms, including the infamous 1040, you will
find; Name: Last, First, MI (for of course Middle initial).
This is true of State forms (here in California anyway).
Sometimes I have seen: Family Name, First, MI.
None of these documents makes the assumption you do, that there is a
connection between First names and any religion. If the good old 1040
asked for a "christian" name there would be a separation of Church &
State issue.

I don't have one. I have a forename and a surname. Christian names are
given at baptism/christening and need not have any relation to the
name on your birth certificate.
