Object: Effectiveness of Remeron at lower dosage?
I have been taking the following meds for some time, for depression and
anxiety:
Remeron (also called Mirtazapine) (15 mg x 3 pills, once a day, at
night)
Desipramine (25 mg once daily, in morning)
Seroquel (12.5 mg at night)
Depakote (125 mg a.m., 250 mg p.m.)
The combination has worked well, but ever since I got on the Remeron
(five or six years ago now), my appetite went up, my weight went up,
and my blood pressure and pulse have been higher. The doctor said the
blood pressure and pulse issues could be partly due to the desipramine
as well, but lower the dosage of that did not help.
I want to try lowering the dosage of the Remeron by one pill per day,
down to 30 mg per day. My questions are:
1. Has anyone who tried lowering the dosage (to 30 mg per day) found
that that helped reduce the hunger, lowered weight (over some time, of
course), and lowered the blood pressure and/or pulse?
2. Did you maintain the beneficial antidepressant effects at the lower
dosage?

The desipramine and your weight probably has more to do with your
hypertension than
does the remeron. Remeron is still therapeutic at 15mg. You *might*
have a lowered appetite with a decreased dose...but physically you have
already
changed and you might still want to eat a lot...and need to eat a lot.
I've been taking Pamelor for years...and when I discontinue it my blood
pressure goes *up* and I feel my heart pounding. Desipramine and Pamelor
are cousins. What does that tell you about these medications?

First off I'm not sure why you are taking 3 15 mgs tabs of Remeron when
you can take one 45 mg. before bed. I always insist on my pdoc giving
me scrips for 30 mgs and then I cut then as needed (less expensive for
me that way). I have been on all three doses depending on the season
and my levels of depression. A dose higher than 45 mgs will not
increase the effectiveness of Remeron.
During the winter, I up the dose to 45 mgs before bed and then lower to
15 mgs in the summer. Weight gain is one of the major side effects.
For me, I have found that the cravings for carbs DOES NOT change with
the upping or lowering of the dose, nor does my blood pressure or
pulse. My BP and pulse rates seem to be geared more toward anxiety
than depression and weight gain.
I suspect the combo of Remeron and Depakote as being your major weight
gain problem. I refused to take Depakote along with Remeron because
both have weight gain as their primary side effects. I know several
people who gained over 30 lbs+ on Depakote alone.

Since my depression seems to be effected by the change of seasons, the
lower dose during the more sunny months remains effective even after
more than 10 years. Sometime you have to experiment with the meds to
get the right "brew."

I have been taking 15 mgs of Remeron in the evenings, and recently
changed it to late afternoon and noticed a lot of difference. I have
also experienced this before. It might be helpful to spread these out
during the day, or ask your doctor about easing up on it. Thanks for
